Free Treatment Plan Template

What is a treatment plan?

A treatment plan is the bridge between assessment and clinical work. It translates the findings from an intake or evaluation into specific, measurable goals that the client and clinician will work toward together, then breaks each goal into concrete objectives and interventions. A strong treatment plan does three things: it explicitly documents what the client and clinician are trying to accomplish (increasing clarity and accountability), it maps the interventions that will move toward those goals (showing medical necessity), and it creates measurable benchmarks so both client and clinician know if progress is happening. Treatment plans are required by most insurance companies before ongoing therapy or counseling can be authorized. State licensing boards often mandate treatment-plan documentation. Supervision standards require that clinical work be grounded in a documented plan. A treatment plan that is specific and measurable becomes the standard against which all subsequent progress notes are evaluated.

What's in this template

This free treatment plan template includes sections for complete treatment planning:

  • Client Identifying Information: Name, date of birth, date of plan creation
  • Problem Statement: Summary of presenting concerns from the intake or evaluation
  • Primary Diagnoses: DSM-5 diagnoses with codes
  • Long-Term Goals: Major areas the client hopes to improve (usually 3–5 goals written as measurable outcomes)

For each goal:

  • Goal Objective: Measurable, time-limited objectives that represent progress toward the goal
  • Specific Interventions: Therapeutic techniques, skill-building activities, modality-specific interventions that will be used to move toward the objective
  • Progress Measures: How progress will be tracked (symptom reduction, functional improvement, skill mastery, rated scales)
  • Target Date: Projected completion timeline for the objective

Additional sections:

  • Clinician Name and Credentials
  • Frequency and Duration of Treatment: Session schedule and expected length of treatment
  • Plan Review and Modification Dates: When the plan will be revisited
  • Client and Clinician Signature Lines: Documentation of consent and agreement

When to use treatment planning format

Treatment plans are required or essential for:

  • All ongoing therapy or counseling after initial assessment (insurance and licensing requirements)
  • Any Medicaid-funded behavioral health service
  • Court-ordered or mandated treatment
  • Residential treatment, intensive outpatient, and higher-level programs
  • Clinician-supervisor agreements about the direction and focus of clinical work
  • Documentation supporting medical necessity on progress notes
  • Any client relationship where clear, measurable goals improve engagement and outcomes

A treatment plan is not optional documentation. It is the foundation upon which all subsequent clinical work is documented and justified.

How to use the template with Scriber

Scriber generates treatment plans from your intake assessment and session recordings. Provide the intake findings and your clinical recommendations, and Scriber structures the plan into goals, objectives, and interventions based on the presenting problems and the client's stated needs. You review and refine the goals for specificity and measurability, confirm the interventions match your actual clinical approach, set realistic timelines, and sign. The plan becomes the reference document for all future progress notes — each note will document work toward one or more of these goals, showing medical necessity and progress. When the time comes to update the plan (required quarterly or when goals shift), Scriber can generate an updated plan based on current session content, tracking which objectives have been met and which goals need modification.
