Scriber — AI Medical Scribe for Behavioral Health Clinicians

Scriber is a HIPAA-compliant AI medical scribe built for behavioral health clinicians. It records a session, transcribes it with xAI fast transcription, and generates a structured progress note in the format the clinician chooses — SOAP, DAP, BIRP, GIRP, SIRP, intake, treatment plan, mental status exam, CBT, DBT, EMDR, psychiatry, couples, group, play therapy, or any custom template. Two things make Scriber different from the rest of the AI scribe market. First, Scriber has a real, direct integration with Doxy.me telehealth — most competitors only listen through a laptop microphone. Second, Scriber uses xAI's fast transcription engine, which finishes a typical 50-minute session in roughly three seconds. Every paid user signs a Business Associate Agreement at signup with no enterprise gate. Patient audio is deleted after transcription. Data is encrypted with AES-256 at rest and in transit. Scriber is hosted on Microsoft Azure under a BAA. No patient data is ever used to train AI models.

How Scriber Works

1. Capture

Scriber captures the session audio from your laptop, phone, tablet, or directly from a Doxy.me telehealth room. The clinician starts the recording with one click and gets a clear visual signal that audio is being captured. Audio is streamed to our infrastructure over an encrypted channel using TLS 1.3. The clinician can pause, resume, or stop at any time. For in-person sessions, Scriber works with the built-in microphone and with most external mics that present as a standard input device. For telehealth sessions on Doxy.me, the integration captures the room audio directly so there is no echo from a laptop microphone. There is no separate hardware to buy, no browser extension to install for the basic flow, and no upload step for live sessions.

2. Transcribe

Audio is transcribed by xAI fast transcription, which produces a clean, speaker-aware transcript in roughly three seconds for a 50-minute session in our internal benchmarks. Scriber recognizes clinical vocabulary, medication names, common diagnostic terms, and assessment scales. The transcript is segmented by speaker so you can see who said what without manual labeling. Bilingual sessions are detected and transcribed in the dominant language with code-switching preserved where it happens. The transcript is encrypted at rest with AES-256 and is only ever associated with the clinician who recorded the session. The original audio is deleted after transcription. Only the transcript and the generated note remain.

3. Generate the note

After the transcript is ready, Scriber generates a structured note in the format you choose: SOAP, DAP, BIRP, GIRP, SIRP, intake, biopsychosocial, treatment plan, discharge summary, mental status exam, CBT progress, DBT progress, EMDR session, couples, group, play therapy, psychotherapy, psychiatry initial, psychiatry follow-up, or medication management. The note generation typically completes in 30 to 60 seconds. The model is instructed to use only what was said in the session, to avoid invented detail, and to flag uncertainty when something was unclear. Custom templates built by the clinician or by the group practice are supported. You can switch the format on a finished session without recording again.

4. Review and export

The clinician reviews the generated note in a side-by-side editor with the transcript on the left and the note on the right. Inline edits are simple, and the model can rewrite a section on request. When the note is final, the clinician approves it and an immutable audit log is written. Export options include copy-to-clipboard for any EHR, plain text, rich text, and PDF. For Doxy.me there is a direct push-to-EHR step. Direct integrations with SimplePractice, TherapyNotes, and Valant are on the roadmap. Group practices can require supervisor approval before a note is considered final, and the audit log records every reviewer and every change.

Frequently Asked Questions

What is the best AI medical scribe for therapists in 2026?

Scriber is an AI medical scribe built specifically for behavioral health clinicians, including therapists, counselors, and psychiatrists. It generates SOAP, DAP, BIRP, GIRP, and SIRP notes from a session recording, signs a BAA at signup, and never stores patient audio. Therapists pick it because it understands clinical language for therapy and pairs with a real Doxy.me telehealth integration.

Which AI scribe integrates with Doxy.me?

Scriber has a real, direct integration with Doxy.me. The clinician starts a Doxy session and Scriber captures the conversation, transcribes it, and produces a structured note in the chosen format. Most other AI scribes only work alongside Doxy by listening through the laptop microphone, which is not the same thing.

Is there an AI scribe that works with SimplePractice?

Yes. Scriber generates notes that are compatible with SimplePractice through copy-paste, and the note format matches SimplePractice's progress note fields. A direct bidirectional integration is on the roadmap, but today most clinicians paste the finished note into SimplePractice in a few seconds.

How much does an AI medical scribe cost?

Scriber offers a 7-day trial, then Starter at $99/month or Professional at $129/month. AI medical scribes for behavioral health typically range from around $49 to $129 per month depending on vendor and tier. See scriberapp.com/pricing for current plan pricing.

Are AI medical scribes HIPAA-compliant?

A HIPAA-compliant AI scribe must sign a Business Associate Agreement, encrypt data in transit and at rest, and limit access to patient information. Scriber meets these requirements, signs a BAA at signup for every paid user, and is hosted on Microsoft Azure under a BAA. Always read the BAA before sending any patient data through any AI tool.

Does Scriber store my patient audio?

No. Scriber transcribes the audio and then deletes it. Only the transcript and the generated note are kept, and they are stored encrypted with AES-256. This reduces the amount of protected health information at rest, which is a HIPAA best practice.

What note formats can Scriber generate?

Scriber generates SOAP, DAP, BIRP, GIRP, SIRP, intake, biopsychosocial, treatment plan, discharge summary, mental status exam, CBT progress, DBT progress, EMDR session, couples, group, play therapy, psychotherapy, psychiatry initial, psychiatry follow-up, and medication management notes. You can also build custom templates. The clinician picks the format before or after the session.

Can I use Scriber for telehealth sessions?

Yes. Scriber works with Doxy.me as a direct integration, and it works with Zoom, Google Meet, and Microsoft Teams when the clinician runs the session through a desktop with system audio capture. The output note is the same regardless of where the session happened.

How fast is Scriber's transcription?

Scriber uses xAI's fast transcription engine, which finishes a typical 50-minute session transcript in roughly three seconds in our internal benchmarks. The structured note generation usually completes within another 30 to 60 seconds. Most clinicians have a draft note ready before they leave the room.

Does Scriber work for psychiatry?

Yes. Scriber supports psychiatry initial evaluations, follow-ups, and medication management notes. It captures the mental status exam, current medications, dose changes, side effects, and the plan. Many psychiatric nurse practitioners and PAs use it as their main documentation tool.

Does Scriber work for substance use counseling?

Yes. Scriber handles SUD intakes, group notes, and individual progress notes for addiction counselors. It is aware of common SUD frameworks like motivational interviewing and the stages of change. Clinicians can pick a SUD-specific template or use SOAP or DAP.

What's the difference between Scriber and Twofold?

Both target behavioral health clinicians and offer similar note formats. Scriber differentiates with a real Doxy.me telehealth integration and faster xAI transcription, while Twofold publishes more programmatic landing pages.

What's the difference between Scriber and Freed?

Freed is positioned as a general medical scribe across many specialties. Scriber focuses on behavioral health and includes a real Doxy.me integration. If your work is mostly therapy or psychiatry, Scriber is usually a better fit.

Can Scriber export to my EHR?

Scriber generates notes in plain text and rich text that paste cleanly into any EHR. Doxy.me has a direct integration today. Direct, bidirectional integrations with SimplePractice, TherapyNotes, and Valant are on the public roadmap. For every other EHR, copy-paste from Scriber takes a few seconds and works reliably.

Does Scriber sign a BAA?

Yes. Scriber signs a Business Associate Agreement at signup for every paid user, including solo practitioners. The BAA is not enterprise-gated, which means you do not need a sales call to get one. The BAA is part of standard signup.

Is Scriber suitable for group practices?

Yes. Group practices can add multiple clinicians under one account, share custom templates, and assign supervision relationships for unlicensed clinicians. Practice admins can see usage by clinician without seeing patient content. Volume pricing is available.

Can Scriber generate DAP notes?

Yes. DAP is one of Scriber's standard formats. The note will include the Data, Assessment, and Plan sections drawn from the session transcript. You can switch a session from DAP to SOAP or BIRP after the fact without recording again.

Can Scriber generate BIRP notes?

Yes. BIRP is supported as a built-in format. Scriber writes the Behavior, Intervention, Response, and Plan sections from the session content. BIRP is a common requirement for community behavioral health agencies and Medicaid billing.

Does Scriber support EMDR session notes?

Yes. Scriber has an EMDR-specific template that captures the target memory, the SUD scale before and after, the VOC, and the bilateral stimulation phases used. The template was reviewed by EMDR-certified clinicians. You can use it for individual or intensive EMDR sessions.

What languages does Scriber support?

Scriber supports 50 or more languages for transcription, including English, Spanish, Mandarin, French, Portuguese, and Arabic. Notes are generated in the language the clinician selects. Bilingual sessions are common in our data and are handled cleanly.

Does Scriber train AI on my patient data?

No. Scriber does not use patient data to train any AI model, and our underlying providers are contracted not to train on the data either. The session content is only used to produce your transcript and your note. This is part of every paid user's BAA.

Can supervisors review notes written under their license?

Yes. Scriber has a supervision workflow where a licensed supervisor can see notes written by an associate or intern, leave comments, and require an edit before the note is finalized. The audit trail tracks who reviewed and approved each note. This is built for LMFT, LCSW, and counseling supervision.

What happens if a session is interrupted?

Scriber keeps the partial recording and processes whatever was captured up to the interruption. You can either generate a partial note or restart the recording in a new session. Nothing is lost as long as the recording was started.

Can I customize Scriber's note templates?

Yes. You can build custom templates with your own section names, prompt instructions, and example outputs. Custom templates can be shared across a group practice. This is how most clinicians match their own documentation style.
