What is a SIRP note?
A SIRP note documents Situation, Intervention, Response, and Plan — a variant of the BIRP format adapted for case management, wraparound services, care coordination, and community support roles where clinicians are often addressing multiple service domains and coordination responsibilities. The Situation section describes the presenting issue or context for the contact — what brought the client to the appointment or what the clinician observed about the client's current circumstances. Intervention, Response, and Plan follow the same structure as BIRP, documenting what the clinician did, how the client engaged, and what happens next. SIRP is widely used in systems of care, youth behavioral health, juvenile justice, and child-welfare-involved services where the clinician's role often includes coordination with school, family, medical providers, and community resources alongside direct clinical work.
What's in this template
This free SIRP note template includes four sections designed for case management and coordination contexts:
- Situation: Current presenting concern or context (housing status, medication adherence, school attendance, family engagement, coordination needs), relevant background, and any external factors affecting the client this week
- Intervention: Actions taken during the contact including counseling, skills teaching, care coordination, provider contact, family engagement, or assistance with concrete services (benefits applications, housing referrals, etc.)
- Response: Client engagement with services, readiness for change, family or system response to interventions, and any barriers or facilitators
- Plan: Next clinical contact, referrals or care-coordination activities, follow-up communication with providers or family, safety planning if needed, and timeframe for next steps
The template accommodates both direct clinical work and the coordination and systems work that defines case management.
When to use SIRP format
SIRP notes are used in:
- Case management and care coordination programs
- Wraparound and systems-of-care services
- Community mental health crisis and stabilization services
- Youth behavioral health and juvenile-justice programs
- Child-welfare and family-preservation services
- Community support programs and recovery coaching
- Any service where the clinician's role spans direct client contact, family engagement, and coordination with multiple service providers
SIRP is particularly useful when a single contact includes both direct work with the client and significant coordination or navigation activities.
How to use the template with Scriber
Scriber generates SIRP notes from your case-management session or contact. Whether you are meeting with a client, coordinating with providers, or supporting a family, Scriber transcribes the session and structures it into Situation → Intervention → Response → Plan, showing both your direct clinical work and your coordination activities. This is valuable in wraparound and systems-of-care roles where the documentation often tracks multiple action items across providers. You record the contact, Scriber structures the note, you review and edit for accuracy and to confirm all coordination follow-ups are captured, and the note is ready to share with the care-coordination team.
