What is a clinical supervision note?

A clinical supervision note documents the content and outcomes of a supervision session between a clinician (supervisee) and their supervisor. Clinical supervision is a distinct form of professional relationship focused on developing competence, ensuring quality care, supporting the clinician's professional growth, and monitoring ethical practice. Supervision differs from therapy: it is not treatment for the clinician, but rather an educational and accountability relationship. Supervision notes create an audit trail of what cases were discussed, what clinical concerns were raised, what feedback the supervisor provided, and what the clinician agreed to work on before the next supervision session. These notes serve multiple purposes: they document the supervisee's ongoing competence and learning, protect clients by ensuring their treatment is reviewed by an experienced clinician, support licensing board requirements for supervision hours, and establish accountability if ethical or clinical concerns arise. Supervisors maintain these notes for regulatory compliance, quality assurance, and supervisee development.

How to use the template with Scriber

Record your supervision session with consent (acknowledge the recording in the supervision contract if required by your license board or organization) and Scriber transcribes the full discussion. Rather than hand-writing detailed notes about each client case while also engaging in active supervision, you have a complete transcript. Use the transcript to edit and organize the supervision content into the note template: extract client names, clinical concerns, feedback provided, and action items; add your clinical observations and supervisory impressions not captured in transcription; document specific teaching points or evidence-based resources you recommended; and ensure the note meets regulatory and organizational standards. This approach lets you stay fully present with the supervisee during the session while AI handles initial transcription.
