Free Biopsychosocial Assessment Template

What is a biopsychosocial assessment?

A biopsychosocial assessment is a comprehensive intake that examines the client's presenting problem through three integrated lenses: biological factors (medical history, medications, brain-related conditions), psychological factors (mental health history, coping patterns, thought processes, emotional functioning), and social factors (relationships, family dynamics, housing, employment, cultural context, support systems). The biopsychosocial model assumes that health and mental health are the product of interactions between body, mind, and social context — that you cannot understand a client's presenting concern without examining all three domains and how they influence each other. Biopsychosocial assessments are required or strongly preferred in many treatment settings: community mental health, residential treatment, clinical social work practice, marriage and family therapy, and any program using an ecological or systems-based approach. They are also mandated by many state funding streams and licensing boards as the standard for comprehensive intake assessment.

What's in this template

This free biopsychosocial assessment template includes expanded sections across all three domains:

Biological Domain:

  • Medical history and current conditions (especially those affecting mental health)
  • Medications and side effects; medication adherence
  • Family medical and psychiatric history
  • Substance use and abuse history with detailed timeline
  • Nutrition, sleep, exercise, and other self-care factors
  • Sensory or neurological conditions; developmental history

Psychological Domain:

  • Presenting problem and symptom history with onset and course
  • Past psychiatric diagnoses and treatment responses
  • Current mental status examination
  • Coping strategies and psychological strengths
  • Trauma history and adverse childhood experiences
  • Learning history and educational achievements
  • Cognitive patterns and self-perception

Social Domain:

  • Family composition, dynamics, and quality of relationships
  • Cultural background, identity, and values
  • Housing stability and living situation
  • Education and employment history
  • Social support network and quality of relationships
  • Community involvement and spiritual beliefs
  • Legal history if relevant
  • Strengths and resilience factors

The assessment integrates findings across domains and creates a comprehensive formulation showing how biological, psychological, and social factors interact in the client's presentation.

When to use biopsychosocial format

Biopsychosocial assessments are required or recommended for:

  • Clinical social work practice (most state LCSW licensing boards)
  • Marriage and family therapy comprehensive intake
  • Community mental health initial assessments
  • Residential treatment or intensive outpatient programs
  • Any program receiving government funding (most states require it)
  • Specialized assessments (trauma, substance use, complex presentations)
  • Supervision or consultation documentation where holistic understanding is essential
  • Court-ordered or legally mandated evaluations

Biopsychosocial is particularly important when a client's presenting problem requires understanding of biological factors (medication effects, medical conditions) alongside psychological and social context.

How to use the template with Scriber

Scriber generates biopsychosocial assessments from your intake session recording. As you conduct the comprehensive assessment — asking about medical history, psychiatric history, family dynamics, substance use, trauma, and social context — Scriber captures the full interview, structures it across the three domains, and creates a draft assessment that is already organized. You then review the draft, add clinical impressions and integration across domains that require your clinical judgment, fill in any specific diagnostic or treatment-planning sections, and sign. For clinicians doing in-depth biopsychosocial work, having the structured transcription and domain organization completed automatically can save one to two hours of documentation time per intake.
