AI Medical Scribe for Primary Care

Scriber's AI medical scribe handles the full complexity of primary care visits — 15-minute slots, six chronic problems, and behavioral-health screening — so you can focus on your patient, not the keyboard.

AI Medical Scribe for Primary Care

Primary care is where medicine happens at full width. In 15 minutes you might adjust a hypertension medication, counsel on weight management, close a gap in preventive care, and notice that the patient's flat affect warrants a PHQ-9. Then you do it again, back-to-back, for the rest of the day. The documentation burden that follows is one of the leading reasons primary care physicians report burnout — and one of the most tractable problems AI can solve.

Scriber is an AI medical scribe built for exactly this workflow. It listens to your visit, understands the clinical context, and generates a structured note you can review and sign in minutes — not after the kids are in bed.

The Primary Care Documentation Reality

The numbers are not abstract. The average primary care visit surfaces 3–6 distinct problems. CMS documentation guidelines for E&M billing require capturing medical decision-making complexity, relevant history, and the physical or cognitive work the visit demands. For a Level 4 visit (99214), the record must support "moderate complexity" decision-making — and that documentation cannot live only in your head.

On top of that, primary care is the frontline for behavioral-health concerns in the United States. More than 70% of patients with depression or anxiety are seen exclusively in primary care settings, never reaching a mental health specialist. PCPs administer more PHQ-9 and GAD-7 screenings than any other specialty. When a patient comes in for a diabetes follow-up and mentions they haven't left the house in three weeks, the visit just became a behavioral-health encounter — and the note needs to reflect that.

Documentation demands in primary care:

  • Problem-list updates across multiple active diagnoses
  • Preventive care tracking (mammography, colonoscopy, immunization status)
  • Chronic-care management (CCM) service documentation
  • Annual wellness visit (AWV) structured review
  • Behavioral-health screening scores with clinical interpretation
  • Referral documentation when a warm handoff is indicated
  • Medication reconciliation notes

Scriber handles all of these within a single visit recording — no switching tools, no separate dictation passes.

What Scriber Captures During a PCP Visit

When you start Scriber at the beginning of a visit — whether in-person or via Doxy.me telehealth — it records and transcribes using xAI's fast transcription engine, one of seven speech-to-text engines available in the platform. The xAI engine delivers results in approximately 3 seconds after audio upload, meaning your draft note is ready before you finish washing your hands.

During the visit, Scriber captures:

  • Chief complaint and HPI — pulled from the patient's own words at the opening of the visit
  • Medication review — changes discussed, adherence issues, side effects
  • Problem-by-problem discussion — each active problem gets its own documentation block
  • Objective findings — vital signs you call out, exam findings you narrate
  • Screening instrument scores — PHQ-9, GAD-7, AUDIT-C, DAST-10 if administered verbally or referenced
  • Assessment and clinical reasoning — the clinical logic you articulate during the visit
  • Plan — prescriptions called in, referrals placed, follow-up interval, patient education delivered
  • Behavioral-health concerns — flags a separate documentation section if depression, anxiety, or substance use is discussed

The transcription engine supports 30+ languages, which matters in primary care practices serving multilingual patient populations. If your medical assistant speaks Spanish with a patient to gather history, Scriber captures that too.

Note Formats Scriber Generates for Primary Care

Primary care visits do not fit a single documentation template. Scriber generates all of the following, and you select the appropriate format when you start the recording (or switch after review):

SOAP Note The standard for acute and chronic problem visits. Subjective captures HPI and patient-reported symptoms; Objective captures exam and data; Assessment captures diagnoses with ICD-10 codes and clinical reasoning; Plan captures orders and follow-up.

Problem-List Update Note For multi-problem visits, Scriber structures the note by problem rather than by SOAP section — matching how most EHRs organize the problem list. Each diagnosis gets its own S/O/A/P block.

Well-Visit / Annual Wellness Visit Captures the structured elements CMS requires for AWV billing: health risk assessment review, preventive care gap closure, functional status, cognition screening, advanced care planning discussion, and a written prevention plan.

Chronic-Care Management (CCM) Progress Note Documents the 20+ minutes of non-face-to-face care coordination time required for CCM billing under CPT 99490. Captures care plan review, medication reconciliation, and coordination with specialists.

Behavioral-Health Screening Note When a PHQ-9 or GAD-7 is administered, Scriber generates a dedicated screening section with score, severity classification, and documentation of next steps — whether that is watchful waiting, initiation of therapy, a referral, or a medication conversation.

Preventive Care Plan Summarizes current preventive care status, gaps, and the actions taken or ordered during the visit — useful for quality reporting and population health programs.

The Behavioral-Health Bridge

Primary care is where behavioral-health conditions are first identified — and often where they are undertreated because the documentation system creates friction. A patient presenting for a blood pressure check who discloses depressive symptoms puts the PCP in a difficult position: the visit time is nearly up, a proper behavioral-health evaluation takes time, and the documentation required to bill for that work is different from a standard chronic-disease note.

Scriber reduces that friction.

When behavioral-health concerns surface mid-visit, Scriber detects the shift in clinical content and adds a structured behavioral-health section to the note. This section captures:

  • Screening scores and their clinical interpretation (PHQ-9 ≥ 10 = moderate depression)
  • Risk assessment language (presence or absence of suicidal ideation, access to means)
  • The clinical rationale for the treatment decision — watchful waiting, pharmacotherapy, referral
  • Warm handoff documentation — the name of the behavioral-health provider, the referral rationale, and what the patient was told

This warm handoff documentation is clinically and legally important. When a PCP refers a patient with depression or anxiety to a therapist or psychiatrist, the record should show that the referral was indicated, that the patient was informed, and that appropriate safety assessment was completed. Scriber captures all of that from what you say during the visit — no separate dictation required.

For practices that screen routinely with PHQ-9 at every visit, Scriber tracks scores over time and flags trends in the Assessment section without the clinician having to manually pull prior scores.

Telehealth and In-Person Workflow with Doxy.me

Scriber integrates directly with Doxy.me, the telehealth platform widely used by independent practices and federally qualified health centers. The integration is real — not copy-paste.

For Doxy.me telehealth visits: Start your Doxy.me session as normal. Scriber connects to the session audio and begins transcribing automatically. When the visit ends, Scriber generates the note in your selected format. You review, make any edits, and copy the finalized note into your EHR. The entire post-visit documentation step takes 2–4 minutes.

For in-person visits: Open Scriber on your phone or tablet before entering the exam room. Scriber uses the device microphone to capture the visit. The same transcription pipeline runs, the same note formats are available, and the same 3-second-to-draft turnaround applies.

Hybrid panel management: Many primary care practices now run mixed schedules — some telehealth slots, some in-person, sometimes same-day switching based on patient need or provider preference. Scriber handles both without workflow changes. The tool is the same; only the audio source changes.

HIPAA Compliance and BAA

Scriber is built for clinical use from the ground up. Every account — including individual PCP accounts and solo-practice NP and PA accounts — receives a signed Business Associate Agreement (BAA) at the time of signup. There is no enterprise-tier gating, no waiting for a sales call to get your BAA.

What this means in practice:

  • No audio retention. Audio is transcribed and immediately deleted. Scriber does not store session recordings.
  • Encrypted transmission. All audio and text data is encrypted in transit (TLS 1.3) and at rest (AES-256).
  • Azure-hosted infrastructure. Scriber runs on Microsoft Azure, which maintains its own HIPAA BAA with healthcare organizations.
  • Immutable audit trail. Every note generated has a creation timestamp and user attribution. The audit log cannot be modified.
  • Zero PHI in browser cache. Patient information does not persist in local storage or browser cache after you close the session.

For primary care practices subject to state-level privacy laws in addition to HIPAA — California, New York, Texas, and others have additional requirements — Scriber's architecture satisfies the baseline. Your compliance officer can request the full security documentation package directly from the Scriber team.

Common Primary Care Questions

What level of E&M can I bill when Scriber documents the visit?

Scriber does not select billing codes — that is a clinical and compliance decision that belongs with the clinician and the billing team. What Scriber does is generate documentation that supports the level of complexity demonstrated during the visit. A 99214 requires moderate medical decision-making; a 99215 requires high complexity. If the visit supported 99214 and the note reflects that, the documentation is there. If it did not support that level, the note will reflect what actually happened. Using Scriber does not inflate or deflate documentation accuracy — it captures what you said.

What specific elements support 99214 vs. 99213?

For 99214, CMS requires moderate-complexity MDM, which means at least two of the following: (1) multiple chronic conditions or a new problem with uncertain prognosis, (2) prescription drug management or an independent interpretation of a test, (3) social determinants of health impacting management. Scriber's note structure captures each of these when they occur in the visit. For 99213, you need low-complexity MDM — two stable chronic illnesses and minimal data review — and Scriber's note supports that documentation equally.

Can I copy the Scriber note directly into my EHR?

Yes. Scriber formats notes as clean structured text. You can copy the entire note or individual sections into any EHR field. For practices using Doxy.me's integrated workflow, the note transfer is even more streamlined. Scriber is EHR-agnostic — it works with Epic, Athenahealth, eClinicalWorks, NextGen, Kareo, Office Ally, and any other system that accepts text input.

What about documentation for CCM billing?

Chronic-Care Management under CPT 99490 requires a documented care plan, monthly clinical staff contact, and 20+ minutes of care coordination per month. Scriber can document each care coordination contact with timestamp, duration, and clinical content discussed — supporting the billing record for CCM.

Does Scriber support behavioral-health ICD-10 codes?

Yes. When behavioral-health diagnoses are discussed, Scriber's note can reference the relevant ICD-10 codes — F32.x for depressive disorders, F41.x for anxiety disorders, F10–F19 for substance use — in the Assessment section. As with E&M codes, code selection is the clinician's responsibility; Scriber surfaces the clinical content that supports the diagnosis.
