What To Do With Your Note Backlog | Scriber Wiki
Last updated
A note backlog — 10, 50, 100+ sessions waiting for documentation — is a real clinical and legal problem. Notes become stale, details fade, and you're working under the pressure of overdue documentation that erodes your focus on current patients.
If you're starting AI scribing with an existing backlog, the tool won't instantly clear it, but it will prevent the backlog from growing while you chip away at it.
The Backlog Math
First, assess how far behind you are.
If you have 50 overdue notes and you're writing 10 new notes per week, you're falling further behind by ~10 notes per week. This is unsustainable. You need to either:
- Write faster (AI scribing helps here)
- Hire someone to help (transcriptionist, associate, admin support)
- Temporarily reduce your caseload
- Combine these approaches
The realistic timeline for clearing a backlog depends on how overdue the notes are. If they're 2–3 weeks old, your clinical memory is usually good enough to write them accurately. If they're 2+ months old, clinical details become fuzzy and notes become less accurate. Prioritize recent overdue notes.
Using AI Scribing to Clear a Backlog
Here's the workflow many clinicians use:
For recent overdue notes (last 2–3 weeks): You can often recall the session content well enough to verbally summarize it while Scriber transcribes. Some clinicians find it efficient to:
- Pull the patient chart and any session notes or progress tracking you took
- Spend 2–3 minutes dictating a summary of the session into Scriber ("Patient reported..." "We discussed..." "Plan is to...")
- Let Scriber's AI layer map that summary into your standard note template
- Review and finalize in 1–2 minutes
This is faster than writing the note from scratch but slower than reviewing an AI-generated note from a real session recording. You're trading recording time (which you didn't have) for dictation time (which takes 3–5 minutes instead of 15–25 minutes to write).
For a 50-note backlog, this approach means:
- 50 notes × 5 minutes (dictate + review) = 250 minutes (~4 hours)
- Versus 50 notes × 20 minutes (write from scratch) = 1,000 minutes (~16 hours)
- Net time savings: ~12 hours
For older overdue notes (4+ weeks): Clinical memory fades. At this point, you have two options:
-
Reconstruct from available data. Review your appointment calendar, patient contacts, treatment plan updates, and any progress notes you did take. Reconstruct the session and dictate it. This is honest and usually sufficient for clinical care continuity.
-
Write a retrospective progress summary instead. Rather than try to reconstruct a specific session, write a brief retroactive note: "Clinical note for 3/15/26 session (note written 4/28/26 — see treatment summary below)." Then document the clinical content you do remember. This is legally defensible because you're being transparent about the timeline.
Do not fabricate details you don't remember. Courts and auditors notice. "The patient reported increased anxiety and we discussed coping strategies" is better than guessing specifics you've forgotten.
Preventing Future Backlogs
This is where AI scribing creates lasting value. If you implement it correctly:
Week 1–2: You're building new habits (starting Scriber before sessions, finishing notes during the day). Your backlog doesn't shrink much yet; you're just preventing it from growing.
Week 3+: Every new session is documented within hours. You're no longer adding to the backlog. Meanwhile, you're chipping away at old notes during administrative time or slow periods.
Month 2+: The backlog is usually cleared because you're finishing 80–90% of new notes between sessions (so the backlog stops growing) and you have time to handle 5–10 old notes per week during catchup periods.
Most clinicians with a 30–50 note backlog clear it within 3–4 weeks of consistent AI scribe use, assuming they also protect some time for backlog work.
Practical Clearing Strategy
If you're starting with a backlog and implementing AI scribing:
-
This week: Stop the bleeding. Start using Scriber for all new sessions. Even if you don't clear the backlog, you're preventing it from growing. This alone relieves psychological pressure.
-
Reserve 1 hour per week for backlog. Pick one afternoon where you have a cancellation or admin time. Spend that hour dictating summaries of 10–15 overdue notes, using Scriber to structure them.
-
Triage by urgency. Prioritize notes for patients you're still seeing (they're clinically relevant). Deprioritize one-time evaluations or discharged patients (still important, but less urgent).
-
Be honest about dates. Every backlog note should have a clear date: "Session date: 3/20/26, note written: 4/28/26." Clinical auditors and insurance payers understand that contemporary note-taking isn't always realistic.
-
Get ahead in weeks 3–4. Once new sessions are documented daily, protect 30 minutes on Friday afternoons for backlog notes. This compounds quickly: 5 notes/week × 4 weeks = 20 cleared notes in a month.
When to Ask for Help
If your backlog is >100 notes or >8 weeks old, consider:
-
Hiring a transcriptionist for backlog only. Not for new sessions (AI scribe handles that), but specifically to transcribe your backlog dictations. Cost: ~$20–30/hour. Clearing 100 notes = 40–50 hours = $1,000–1,500. This might be worth it to relieve the pressure.
-
Medical scribe or documentation specialist. Some practices hire an admin to sit in sessions and take structured notes, which you then dictate summaries from. This is more expensive ($18–25/hour) but offloads the session note-taking entirely.
-
Working with your EHR vendor. Some EHRs have backlog-clearing services or templates for retrospective documentation. Ask your vendor if they have guidance.
The Bigger Picture
A backlog is a symptom of a system that isn't working. AI scribing fixes the system going forward, but it doesn't erase the fact that you got behind. Use backlog clearance as a forcing function to:
- Audit your schedule. Are you seeing too many patients? Do you need to reduce caseload slightly to allow documentation time?
- Review your templates. Are your current note formats too detailed or too loose? Optimizing templates saves time on every future note.
- Set a documentation policy. Decide: notes must be complete within 48 hours, or 1 week, or by Friday EOD. Communicate this to your team.
AI scribing handles the execution; you handle the policy. Together, they prevent a backlog from building again.
